External Job Description: APS Healthcare is seeking a FT Clinican to function as a Team Leader for our SMI Programs. Position willl be based at our Nimitz office in Honolulu, HI.

Position Summary:
Supervises Field Based Care Coordinator Teams, delegates duties to Care Coordinators and Outreach Specialists, and responds to operational issues/concerns while promoting quality and productivity in the administration of behavioral health services.

Essential Functions:
1. Manages the day-to-day operations of work assignments for direct reports (care coordinators) to ensure adherence of standard goals/metrics for quality and performance. Develops and provides mentoring to Care Coordination Teams to ensure that participants are receiving quality services in an appropriate and timely manner. (30%)
2. Manages a caseload and oversees the coordination of all CM services to include the development of an Individualized Recovery Plan (IRP), completion of the case management assessment, acuity determination, utilization review as needed, and regularly reviews member charts to ensure appropriateness and quality of care.. (30%)
3. Provides on-going clinical supervision and direction to care coordinators in their coordination of care activities. Conducts clinical case reviews with team members. (20%)
4. Conducts FUH (follow up after hospitalization) visits to members & participates in the on-call rotation (10%)
5. Assists the Clinical Services Supervisor with development and on-going refinement of tools, workflows, policies and procedures used by APS Healthcare s SMI Programs (10%)

Education:
Masters Degree in Behavioral Health field or Masters Degree in Nursing required.
Active and unrestricted Hawaii Licensure as a health professional (LCSW, LMFT, LMHC, APRN)

Experience:
Minimum 3-5 years experience in a behavioral health setting required, preferably with the SMI population in a field based capacity.
Minimum 2-3 years management experience required.
Excellent assessment skill; strong written and verbal skills
Strong Leadership skills
Proficient at Microsoft Word and Excel
