The HH Medical Assistant (PCMH) assists with administrative functions and supports care coordination of physician practice members within the patient center medical home model.
Supports ongoing physician - member relationships by providing educational and operational support to improve the quality of the member experience
Provide direct member outreach for quality metrics as determined by HMSA and Healthways.
Assists with office administrative functions (as needed) including member follow up with Healthways programs.
Scope
Supports member interaction to improve HEDIS metrics: example: Scripted member reminder phone calls.
Supports the physician and staff in care coordination activities: example: Appointment scheduling and care gap tracking.
Job Complexity
Works on assignments that are moderately difficult, requiring judgment in resolving issues or in making recommendations
Works in support of the physician practice to coordinate the activities that align with goals of HMSA/Healthways Hawaii PCMH model.
Supervision
Normally receives little instruction on daily work, general instructions on newly introduced assignments.
Works in the physician practice with regular oversight by the Provider Support Manager and PCMH Care Liaisons as agreed based on contractual agreement with HMSA and Healthways.
Skill
Has substantial understanding of the job and applies knowledge and skills to complete a wide range of tasks.
Location
Position based in Healthways Hawaii Kaimuki with field work to support PCMH agreement.
Qualifications
Experience
Medical Assistant certification
Generally requires a minimum of two to four years of physician office or related experience.
QUALIFICATIONS:
1. GED or High School diploma supplemented by technical medical knowledge, specialized administrative and/or computer courses. College course work is desirable.
2. A minimum of 2-4 years experience in the healthcare field, such as Medical Assistant, Medical Technician or health care assistant.
3. Ability to work effectively with all levels of management and other colleagues; demonstrate initiative, mature judgment, and customer service orientation; and anticipate needs of principals.
4. Possess computer competencies to include word processing, spreadsheet, graphics, and data base management.
5. Language Skills: Ability to communicate with members, other members of the team, physicians and Plan representatives. Effective oral and written communication skills.
6. Math Ability: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
7. Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exits. Ability to interpret and organize data in an effective and useful manner; ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
8. Computer Skills: To perform this job successfully, an individual should have basic computer skills and be familiar with a Windows operating system.
